Please call our office during office hours so we can make an appointment for you. We will make every effort for you to be accomodated in a timely manner and address your concerns.
Please bring your insurance card and photo ID with you to your appointment and arrive 30 minutes prior to fill out forms. If you have insurance through another party, like a parent or husband, you will need to bring their insurance information, name, date of birth, social security number and address. Make sure to inform the receptionist when making your appointment, if your insurance is though another person or who is your responsible party.
If you have a favorite pharmacy or one close to you, please bring the name and address of that pharmacy so we can E-prescribe medication to the pharmacy of your choosing.
In addition, if this is your initial appointment, bring all the bottles of medications you are currently taking and any medical records you may have, as well as the name and location of your prior physician.

Meet Gil Y. Cortes, MD
Dr Cortes is a Board Certified Dermatologist with years of experience in his field. Originally from Aguadilla, PR, he is happy to be back at home and provide any service you may require from him.
After completing medical school at Ponce Medical School in Ponce, PR, Dr. Cortes went on to specialize in Dermatology at the Medical University of South Carolina, in Charleston, South Carolina. He then worked at The Diagnostic Clinic in Largo, Fl for several years.
He is skilled in treating all manner of skin disorders, skin cancer excisions and repair, Psoriasis requiring treatments such as Humira and other Biologics, cosmetic treatments such as Botox or Restylane as well as lasers and hair implantation techniques, among others.
Meet Cristina N. Cortes, MD
Dr. Cristina is a Board Certified Internist with years of training and experience in her field. Of Cuban descent, Dr. Cristina is fluent in Spanish and English.
She completed medical school in the Dominican Republic in San Pedro de Macoris, at the East Central University. Afterwards, she went on to specialize in Internal Medicine in Mt. Sinai Hospital in Miami Beach, FL. She was a Clinical Instructor and Academic Hospitalist for The Medical University of South Carolina in Charleston, SC where she taught medical students as a general attending for several years. Afterwards she worked in The Diagnostic Clinic for several more years in an outpatient setting managing general medical concerns, preoperative consults and follow up care for all manner of medical problems.
Dr. Cristina manages a outpatient practice for all types of medical concerns, and a separate weight loss clinic where she provides individualized attention to anyone who might request it.
We accept several different insurance plans listed below. If you don't see your insurance listed, you will be charged for your visit. You may request a receipt for your visit and submit your insurance claim personally to your insurance provider. Keep in mind, all insurances are different and coverages may vary or change at any time. For questions regarding your insurance, please contact your insurance provider directly.
